During calendar year 2011, the Avenues Health Plan will be underwritten by Humana. Any employee who is classified to regularly work a minimum of 32 hours per week and has completed 90 calendar days as an Avenues' employee, may be eligible for coverage under this plan.

Enrollment for eligible employees would begin on the first day of the month immediately following the 90th day of enrollment eligibility. Check with your immediate supervisor if you have any questions about your eligibility.

Currently, a per paycheck deduction of $15.00 is paid for employee only basic HMO coverage.

Higher HMO, PPO, Family and dependent coverages are optionally available through payroll deduction at the employee's expense. Please review the health deductions page below for specific information.

Plan underwriter, co-payments, employee contributions to premiums and other terms of this plan are subject to change at any time.
