Transition from Educational to Adult-based Services
The
Individuals with Disabilities Education Act of 1997 (IDEA 97)
requires transition planning for all students who have an Individual
Education Plan from the age of 14
through the end of their schooling at the age of 21-22.
Transition services are a coordinated set of activities that include
instruction, community experiences, related services and the
development of employment and other post-school living objectives.

meet these requirements and to ensure that transitioning students
have the best possible outcomes upon leaving the educational system.
